In order to schedule an appointment, we require a credit card to be kept on file. Payment is collected at the start of the appointment. In order to cancel, Mass Functional Medicine must receive notice 3 full days by voicemail or text message to our office number 845-623-0047. If the patient appointment is on Wednesday at 10:00 am, the patient must cancel by the Friday before at 10:00 am. This allows us enough time to offer this appointment to our waitlist. Our cancellation policy is at the bottom of our appointment confirmation emails.
Mass Functional Medicine plans for and aims for punctual appointment start and end times. If a patient is running late, Dr. Mass will start the appointment upon arrival, but must end on time in order to protect the patient schedule for the rest of the day. If Dr. Mass is running behind schedule, we make every effort to notify the patient as soon as possible. If Dr. Mass is running late, he will ONLY make up appointment time that exceeds delays beyond 10 min. Only if there is room in his calendar, Dr. Mass will extend the patient appointment beyond the scheduled end time. Otherwise, our office will add the time to be made up at the next appointment. The office manager logs and handles all schedule adjustments, and must be notified by the patient immediately.
All patients joining our practice start with a 90 min consultation. We require medical records, previous lab results within the last 6 months and the MFM new patient questionnaire to be submitted to us at least 3 full days before your appointment. After the new patient consultation, our patients' follow-up visits should be scheduled 4-5 weeks from the first consultation.

Please plan accordingly for any medication refills to avoid disruption to the patient's medication schedule. For the first few months the follow-up visit will be 60 min. This allows enough time to thoroughly evaluate the patient from the last visit and diagnose any symptom changes. Dr. Mass will revise the patient protocol, if needed.
Dr. Mass is available for consultation on various health topics. This appointment is NOT to diagnose or treat, but to collect Dr. Mass medical opinion. Please contact our office to schedule an appointment. Be prepared to submit medical records or any documents for review at least 3 days prior to the scheduled appointment.
Once a patient is stable on a the protocol, and symptoms have improved - follow-up visits are reduced to 30 min. Dr. Mass will revise the patient protocol, as needed. This appointment may also be scheduled for reviewing lab results with Dr. Mass, that require more time for discussion and for questions to be answered. This appointment type may also be scheduled to review lab results or patient questions.

Dr. Mass is current with his medical license in France and Europe. Same rates apply.
Dr. Mass is a U.S. Civil Surgeon. He offers the Immigration and Naturalization Services Medical Exam and completion of the I-693 medical form. Additional fees are the cost of the bloodwork (if no insurance coverage) and the cost of any required vaccines. Contact our office to schedule an appointment. A medical letter is only available for patients currently under Dr. Mass' care and based on medical information evaluated and documented by Dr. Mass in patient appointments. Current patients have had a recent visit with us in the last 30 days may request medical letters . There is a fee for this service as the review of medical records by Dr. Mass is required.
